Problem description

Chronic diseases is directly connected the elderly people especially from the poor background. They cause the biggest risk of premature death or being disabled due to the risk of physical and social health. The demographic is towards the poor people and the homeless are constantly burdened by the exposure to risks and lack of access to primary health care. Some of the exposure relating to chronic conditions includes smoking, drugs and being exposed to harsh weather conditions or even lead paint poisoning. As a result, they might be affected by chronic diseases such as cancer, diabetes (Morewitz & Goldstein, 2007, p.56), heart diseases, stroke, Alzheimer diseases, pulmonary conditions, and internal organs failure such as the liver and the kidney. More than half of the population in Illinois is affected by such conditions which reduces the quality of life and shortens the period because it is the leading cause of death.

Chronic disease creates a very wide economic constraint which might be strain to the caregiver. Studies have shown that more than twelve point five billion dollars are used in a year in Illinois to treat chronic diseases. As a result, the financial burden may cause absenteeism in the place of work, less productivity for the employees hence reducing the amount of income of the employer. In essence, it can be easy to prevent chronic diseases. This can be done by constantly checking one’s health and changing one lifestyle choices. A healthy diet can go a long way in preventing some chronic diseases such as diabetes, stroke and even heart diseases. Also, the combination of a healthy diet, exercise and avoiding tobacco can be a preventive measure for up to forty percent of cancer diseases. The Illinois Department of Public Health has come up with programs aimed at reducing the burden of chronic diseases. This department is dedicated to address the issue of chronic diseases and the burdens related to the same. They are able to achieve this by having a public health policy that entails of communication of the preventive measures that the population can take to prevent chronic diseases. There is also surveillance and epidemiology in a bid to evaluate, mobilize and create awareness of effective management of chronic diseases. There are some causes that majorly affect the rate of chronic disease in Illinois. Some of the causes include poverty, some of these people who are highly affected do not have sustainable household that can provide the basic needs required to live a comfortable life. A behavioral characteristic which includes regular physical activities and the lifestyle is also another cause. There is a high chance of obesity and high cholesterol levels due to some eating trends. Some extreme health conditions are also a major cause of chronic diseases. These can be characterized by how well internal organs operate and how often one can access health care for checkups. It is important to have regular checkups, do regular physical activities and maintain a healthy diet in order to avoid some of the chronic diseases related with such factors.

Evidence-based practices associated with chronic diseases are relevant to people living with these diseases and some preventive measures are important. These practices can significantly reduce the burden that comes with chronic diseases. The community health departments can access a problem and develop an appropriate policy to control or give solution to the problem. Lastly, they should ensure that the policies or solutions reached are easily disseminated and effectively implemented to the population. The best way to make a decision on evidence-based practices is to ensure that there is the availability scientific evidence present, how they can be applied and improvement seen by engaging the community. There are some interventions that can be used to prevent chronic diseases or assist people living with chronic diseases. For instance, it is important to have a healthy lifestyle, frequent screening and vaccinations or care to those people living with chronic illnesses. People living with chronic diseases should be responsible to adhere to treatment and increase the effort and willingness to live with some conditions. Preventive inventions are strongly recommended to the general population which can affect the disease process, how the diseases progress and any complications available. Generally, there are some preventive measures that are common such as physical activities. They have some benefits to chronic illnesses because they reduce the chances of having a cardiovascular disease (Morewitz & Goldstein, 2007, p. 125), some cancer, and diabetes and generally improve the functioning of organs in the body. They have a tendency of improving one’s physical fitness and also determine the functioning and lower the progression of illnesses. They improve the chance that one can be fatigued from medication and treatment and reduce the chance of depression for the patients. However, there are few evidence-based community programs that support the increase of physical activities for people with chronic diseases even though they have been successfully implemented and have effectively shown the benefits of the activities. Another evidence-based intervention that should be highly encouraged is the use of tobacco in the community. Many individuals with chronic diseases are as a result of smoking. It is important to sensitize the benefits of smoking cessation for individuals living with chronic illnesses. The more a person smokes the higher chance for a chronic disease to progress and attract complications of some diseases. Community health workers have the obligation to discourage the use of tobacco to the population which will reduce the exposure of secondhand smoke which is equally harmful to the population. Recommended evidence-based interventions for tobacco may include media campaigns, increment in acquiring the product and population education against the use and damages caused by tobacco whether for smoker and for secondhand smokers. The people affected with chronically ill diseases are affected by multiple complications throughout the period. Every individual with any condition should be encouraged to go for screenings frequently and also vaccination against common viruses that can be prevented.

Social Media Campaign

Social media campaign aims to create and integrate concepts or information that will in turn influence individuals or a community for a greater good of the population. In community health, social media campaigns include a specific range of information that will assist the community to change their behavioral aspect. There are some social media mediums that one can use for the campaign. They include blogs, podcasting, photo and video sharing, and social networks such as facebook, twitter, LinkedIn, MySpace, intsgram and whatsapp (Meskó, 2013, p.44). This is after they overtook the commonly used mediums which included television, radio and print. The main objective of social media campaign is to provide and allow caregivers a platform to communicate quickly and simply to large number of patients. This has enabled the easy interaction and quick response. It also ensures that a large audience has been reached and information shared widely. Another objective is to ensure community health department share their thoughts and patients pursue their interests. It is also easy and efficient to promote the well being of the community and educate the community on issues that are health related (Meskó, 2013, p.123). It is easy to access information from social media for patients because they can save on time intended to go for consultations.

The use of social media as a way of communication gives the provision of information rich alternative to advertising. Consumer market for social media has been highly accepted to connect easily with the public who are willingly looking for information. Social media promotes statements; it has enabled nurses to promote community wellness by educating the audience. They can be used in various ways to educate like common conditions and disease management of a vulnerable population. Social media has been effectively used to educate matters on community health. Community health nurses have used social media as an effective tool to educate consumers. For example, a post published on facebook can contain information and advice that is helpful, with the commentary section, it is easy for the caregiver and patient create rapport and even when necessary use private means to communicate. Social media has been helpful in creating common ground for patients who are struggling with the same kind of struggles. When patients are diagnosed with some illnesses, they tend to be depressed and discouraged. Community health providers have opened social networks that bring together individuals with similar conditions as a way of coping with the emotions. Information sharing has been highly encouraged by social media while giving a speedy chance of research and participation. This is due to the ability of information sharing and the influence social media has to the masses. Social media enables the ability to publish time-sensitive information and manage misinformation by sharing relevant and accurate information (Meskó, 2013, p.111). Content development is the core success of community health workers to actively use social media to their advantage. Easily understandable content catches the reader’s attention and it is easily sharable and recommendable to others. Social media has managed so far to keep the conversation going for practitioners and patients.

The most appropriate social media platform that will be of great help and value for community health nursing is a blog site, facebook, video sharing and world wide website. For effective communication the practitioners should have skill that develops successful media campaigns (Meskó, 2013, p.40). Such skills include knowing their target audience, have facts right based on any topic they aim to discuss with the audience. Having campaign materials, research and evaluation tools, and risks and crisis communication guides and lastly, social media tools and templates. My recommendations for the above social media sites are due to their ability to have an interactive communication where information is passed which evokes the audience to communicate.

The target population will benefit from the social media campaigns because of the increased interactions with others. Social media has become a source of support for patients diagnosed with chronic diseases as they engage with others with similar conditions. The more the information on specific condition social media has the easier it is for an individual to cope with their diagnosis. There is a chance to learn more on the condition from different sources. Another benefit social media has done is increase the accessibility and widen the access of relevant information according to a patient’s need. This creates expansion of knowledge for both patients and caregivers who consult other physician from all over the world. It is also an easy way to create moral support to each other as patients across all walks of life. This has influenced a positive health outcome due to patients receiving support from family and peers. Social media has a potential to influence health policy in the future as to how the communication is done, and the response given to patients. This information collected can be analyzed and give possible guidelines for influencing policy making.

The best way to implement social media tools for health marketing is to maximize the impact made. This can be done by creating content that can be easily shared across all the social media platforms. The best way of implementation would be to engage the patient in real time when they need the information. In this one should ensure that they address the patient questions, concerns and interest on time and avoid delayed replies and information. Health care providers can also take advantage of social media to encourage patients to collaborate with physicians.

Stakeholders have the role to assist a group of people achieve or complete a project. A stakeholder is someone who has a vetted interest on the project you are working. They can be individual or organizations. They have a responsibility to identify constraints and provide time and cover the cost of meeting the management process (Morewitz & Goldstein, 2007, p.15). The stakeholders should also execute the project management plan and ensure accomplishment of the project scope. Some potential stakeholders that could formed to aid in the implementation of such a media campaign are online groups, blog readers, website managers, non government organization that have the same goals as your team (Meskó, 2013, p.63). There is a need to have a specific timeline for the success of implementing a media campaign. No specific timeline can be measured however for a media campaign to be successful one must put into consideration key skills required in a team. The organization and team management should be perfect, the role of writing, editing, graphics, networking skills, customer services, and advertising expertise should be given to team members who have the knowledge and experience thus the time used will be short to implement. A team can be able to evaluate the effectiveness of the campaign from the feedback gotten from social media. Whether positive or negative feedback it should give the team a rough idea on how far the implementation is acceptable and what can be changed to make it more effective. It is important to measure the tools necessary for your campaign in order to review the campaign (Meskó, 2013, p.82). Such tools include ongoing monitoring of the activities taking place in the social media platform and campaign focused analysis of the clear information you are giving out and the response. The cost of implementing a social media campaign can vary from as low as $4000 up to $20000 a month. This is highly dependent on the social engagement, content creation, paid adverts and analysis.

Social media marketing and campaigns support the community health sector by promoting health services to the population. This is achieved when they educate the public on awareness of matters on health and support the existing relationships between healthcare providers and the patients. Social media campaign can also apply in the future nursing practice as a library and reference on health concerns and issues. Practitioners and patients have a common ground to achieve any information related to health that can be helpful to them.
